Organisational Structure of Geodrill Limited Introduction Geodrill Limited is an exploration drilling contractor, which was incorporated in the Isle of Man in June 1998 and centers its attention on the mining industry in West Africa. It operates in 3 West African countries, which are Burkina Faso, Ghana and the Cote DIvoire through an entirely owned subsidiary, Geodrill Ghana Limited. The Head Office is located in Accra, Ghana and manages operations throughout the West African region. However, there are only two operating entities, Geodrill Ghana Limited and Geodril CI SARL in Cote DIvoire . Geodrill Ltd (Isle of Man) operates contracts in Burkina Faso. The two entities are whole owned subsidiaries of Geodrill Ltd ( Isle of Man). Geodrill Ltd. has 2 issued shares owned by Blue Croft Limited and Red Croft Limited. All drill rigs were bought new except ore 650/1 which was bought second hand in the year 2000. Geodrill started operations in Ghana in 1998 with one UDR 900 multi-purpose drill rig and in 1999 added another. At the end of December 1999, Geodrill owned 10 UDR multi-purpose Drill rigs all with boosters, auxiliary compressors, modern fleet of MAN trucks, track support vehicles and light vehicles. Geodrill also owns three core only Sandvik DE 710 crawler mounted drill rigs and two Auztex Air core truck mounted rigs. Current expansion in 2010 includes two additional rigs bringing total fleet to seventeen. It is envisaged that by the end of 2010 the fleet size in terms of drill rigs will be twenty. The rigs to be added in 2010, while still of the UDR make (have recently changed to Sandvik) depart from existing strategy by being specialized drill rigs. Two of the rigs will offer RAB services while the other offer Air Core. The entire maintenance operations of Geodrill Limited is carried out at Anwian Kwanta a suburb of Kumasi in the Ashanti Region of Ghana. It is based on a 10 acre land which has a modern workshop, offices and accommodation for both expatriates and nationals. The workshop strictly adheres to world class safety standards. Our health and safety and environmental program was designed by Tracmin to at a minimum comply with Australian safety standards. Ownership and Directors Geodrill Limited has a Trust set up by Dave Harper, currently the Managing Director of the group. He is also the Chairman and CEO of Geodrill Ghana Limited, the operating entity in West Africa. The Trust is held by the City Trust firm based in Douglas, the Isle of Man, on behalf of the Harper Family Trust of Perth, Australia. It has 2 issued shares. The other director of Geodrill Ghana Limited is Terry Burling who is also the Operations Manager of the firm. Figure 1: Organisational Structure of Geodrill  [1]   The Organisational Structure Management Dave Harper is the Managing Director and CEO. He has about 30 years of experience in the drilling industry having worked both in Australia and West Africa. He started working in Ghana in 1991 as General Manager of SMS, an exploration drilling firm. Mr. Harper, a driller by profession, resigned in 1996 and set up Geodrill 2 years later. The CEO is directly responsible for project flow and contracting with clients, client relations, invoicing and acquisition of major equipment. He also leads in setting corporate and business strategy. He is assisted by 2 departmental managers. Terry Burling is the Operations Manager with 20 years of experience in the drilling industry. He previously worked with SMS in New Zealand, Australia, Papua New Guinea, Indonesia and in Ghana. Fortunately for the company, Terry Burling is an expert in both the mechanical and exploration aspects of the profession. His skills span drilling, mechanical engineering, electrical engineering and fabrication. Greg Conroy is the FA Manager. He has worked in that capacity at several exploration drilling and mining firms including SMS (Ghana), Equigold CI SARL (Ivory Coast). He has over 20 years of experience in the industry. Conroy started working with the company in December 2008, having taken over from a Ghanaian who had been in the position since Geodrill was established and also previously worked with SMS. The FA Manager is directly responsible for managing relationships with all external stakeholders except clients. The Structure of the Firm Geodrill has about 120 staff organized into 2 departments, Operations and Administration. There are 4 sections under Operations: Exploration, Maintenance, Stores and HSE. Exploration performs the drilling required by clients; the core activity of the company. The Exploration workers are organized in crews of 4 per rig per shift of 12 hours, therefore 8 per workday. A Site Supervisor manages all aspects of the project being undertaken including a cash expense budget, accommodation, liaises with the clients representation usually a geologist, and supervises the crew. If its a large project with 2 or more rigs present, a store is set-up and stocked with consumables. In this case too there may be one or more supervisor. The Exploration staff typically constitutes about 80% of the staff strength. Maintenance provides engineering back up services for repairs and maintenance. This section also fabricates a lot of equipment including heavy duty, pneumatic vehicles. They work out of one of the most advanced workshops in West Africa and drive out to offer on-site support in their modified and well-equipped Land Cruiser pick-ups. The Store section manages the stock of consumables, fuel and spare parts. Though many firms in the industry treat stock as finance and accounting function because of its effect on profitability, Geodrill has placed its Store as Operations to better ensure closer integration with the core functions of the business. Decisions on purchases are made with paramount regard to the necessity of the item for project execution rather than cost or any other consideration. Requests routinely originate from the Supervisors and are validated by the Operations Manager. The Health, Safety and Environment section oversees the design, implementation, monitoring and evaluation of Health, Safety and Enviornment standards that, at a minimum, are required to conform to Australian mandated standards. The Australian standard is regarded as the most stringent for drilling firms globally and much higher than what is required in Ghana. Setting that as the minimum standard is a purely discretionary decision by Geodrills Chief Executive Officer. The Administration department is made up of Accounts, Human Resource, Information Technology and Logistics. The Accounts section is responsible for all financial reporting including management reports and statutory reports. Geodrill tracks income by drill rig and by project. Major direct expenses such as fuel are tracked by drill rig and project. The Accounts section also manage the monthly cash budget. Human Resource section coordinates with departmental managers in the recruitment process, the setting of salaries and promotions. Salary levels are determined according to the hierarchy (organizational structure) and each band has 3 sub-levels. Junior employees earn a basic salary and overtime for work done beyond 8 hours in a weekday. For senior employees, a fixed salary that consolidates both a base pay and overtime is determined and their pay does not vary by the hours done. The section prepares the payroll and executes payments. The Logistics section act as a go-between with Stores, and the rest of the company to ensure that procurements, imports and exports and immigration requirements for expatriate workers are smoothly executed. Services Geodrill provides Reverse Circulation and Core Drilling services. The object of all prospective drilling is to get samples from different depths below the surface. The two basic methods are cuttings recovery and core recovery. Reverse circulation drilling falls in the first category and diamond core drilling in the second. Reverse Circulation Reverse Circulation is the method of choice for obtaining consistently high quality rock chip samples with speed and efficiency. Geodrill provides reverse circulation drilling using the latest Face Sampling hammers and equipment drilling holes with 140mm diameter to depths of between 50 400 meters. In terms of pricing, this drilling methodology is positioned in the middle. It is more expensive than RAB and Air Core drilling but less costly than diamond drilling. It is the most commonly used method. It collects samples from depth by pulverizing the rock which is then blown up through an inner tube and collected in a sample bag. Diamond Core Drilling This technique is used to drill to about 1500m deep. A diamond-impregnated drill bit cuts a core of rock hence the name Diamond core. The core sample is removed at intervals and catalogued. This is a much more expensive drilling method. Directional Drilling Geodrill provides the Deep Directional drilling using Down Hole Motor and Wedging; to control deviation so target is achieved, to intersect targets below inaccessible locations, to alter course of a drill hole to intersect target and to allow multiple target intersections by deviating branch holes from a parent hole. Two other types of drilling, common in the industry but not previously offered by Geodrill, are RAB and Air core drilling. These are most ideal for shallow depths, are speedier and cheaper for clients. THE EXPLORATION INDUSTRY IN WEST AFRICA Market and Industry Geodrills current client list includes Newmount Ghana Gold, Golden Star Resources, Keegam Resources, Perseus Mining, Azumah Resources, Castle Minerals, Lihir Equigold, in Cote dIvoire, Ampella Mining and Gryphon Minerals both in Burkina Faso. Geodrill is currently the market leader in Ghana with about 35% of the market share. In 2009, 50% operating capacity in Ghana, 20% in Cote dIvoire and 30% in Burkina Faso. Geodrill hopes to increase operations in Burkina Faso in the near future as that industry is less saturated than Ghana in terms of competition and there are a lot of contracts readily available. For 2010, the order book is full including 2 new rigs on order. Geodrill is still receiving invitations to tender on contracts all over West Africa in Gold and base metals. Geodrills competitors in Ghana are Boart Longyear, AMS exploration, Pontil Minerix, Drillmasters and Burwash. In Cote dIvoire, their competitors are Boart Longyear CI, Ore-ex and Drill ex. In Burkina Faso, its Boart Longyear BR and West Africa Drilling Services. Current industry challenges include price competitiveness, and the labour market beginning to tighten. Opportunities include gold and commodities boom, an influx of active exploration companies into West Africa and improving political stability in the region. The mining, energy and infrastructure markets are expected to see the hightes neer to medium term growth in the sub Saharan African region. In general, mining companies with a higher percentage of long term renewable contracts are viewed as being lower risk and Geodrill prefers to deal with such clients. Analysis of the Industry There is no substitute for exploration drilling services in the mining industry. The exploration process progresses from initial studies of the geological structure of an area, a history of mining and other indicators of prospective potential. However, actual samples must always be collected both from the surface and at depth to determine the actual presence of mineral. This collection of samples from depth can only be done by drilling down into the ground. The threat of new entrants probably offers competitive pressure to existing firms in the West African market. The mining industry is made up of many small firms but dominated by a few very large firms. Exploration drilling, though it has a tangible end product in the form of samples, is really a service and depends as much on relationships as on the quality of the sample. It is a capital-intensive business but any entity that can get access to about $3m in start-up capital can start with a second hand rig, some spare parts and consumables. The exploration drilling firms usually operate under the umbrella of the client and so are indirectly regulated hence regulatory issues will not hinder a start-up. The exploration drilling business is characterised by short drill programmes (contract duration of 3 months to 1 year). Each drill programme can be properly viewed as a real option, and determines the next step for the client. The level of activity is driven by the ease of fundraising on the capital markets and the price of gold as it determines the value of any find. The drilling service is homogenous in nature, hardly differentiated except by the speed and efficiency of execution and the safety record of the drilling firm. There is not much collaboration among the firms. Suppliers of major inputs such as drilling consummables are quite powerful when the industry is booming. When the price of gold falls or the capital markets are not doing well, there is usually an excess of serviceable second hand equipment available. This dilutes the power of the suppliers somewhat. The sources of consumables are many hence the suppliers here are much less powerful than the suppliers of equipment. There are two types of clients: Mining firms and exploration firms. The mining firms have operating mines and earn revenue from selling the gold that they mine. The exploration firms hold concessions and carry out drill programs to determine their potential. Exploration firms do not earn operating revenues; they earn a return eventually by selling the concession if it is proven to have the potential to yield some gold. Therefore, exploration firms finance their drill programs by raising equity on the capital markets. The clients are very powerful and price sensitive. Drill programs tend to be one of the largest costs of the client. For both types of clients, it determines the life and value of the business. Accounting Policies Drill rigs undergo major rebuilds to extend their life span every 3 5 years. The cost is capitalized. Depreciation rates are 10% for drill rigs, 20% as plant and equipment, 20% on motor vehicles, 20% on furniture and fittings and 5% on land and buildings. Currency of record is the US dollar. Audited financial statements are submitted to the internal revenue service and taxes are paid in US dollars. Geodrill is permitted by the Minerals Commission and Bank of Ghana to receive at least 50% and Bank of Ghana to receive at least 50% of its drilling income in US dollars. Financial year ends on 31st December. Organisation Strategy From inception, management decided it did not want a Workers Union. Management instead formed an informal workers committee which is made up of senior employees and who are nationals for that matter. Management then acts as a union and seeks to ensure the general welfare of workers in consultation with the workers committee. This is itself is unique in an industry where unions are common. 2. Geodrill operates only in West Africa whereas its key competitors operates globally. From Ghana the chairman who also doubles as the chief executive officer oversees everything. This gives Geodrill the advantage to follow the market more closely and adopt quickly. For its competitors, there might be a general manager heading a particular region who will lack final authority on decision making. 3. Geodrill adheres to a strict healthy, safety and environment policy and as much as possible used brand new equipment which are serviced regularly and kept in top shape by the operations department from its workshop which is fitted out with the most advanced equipment needed. Competitors often deploy other rigs that have been in service in other parts of the globe and this gives Geodrilll an edge over competitors when it comes to contract execution. 4. Geodrill has more nationals in their top hierarchy than its competitors. Locals are found in management positions usually reserved for expatriates among their competitors. This enables management to keep the wage bill lower than that of their competitors because nationals severally earn lower than expatriates while at the same time motivating nationals to give off their best because they know the top positions are not kept for only expatriates. 5. Geodrill pays higher than market rates and keeps job content interesting so as to attract the best men available and also maintain them. To lay off full time permanent employees however, is difficult under the labour laws of Ghana and other West African states hence Geodrill keeps 80% of nationals on 2 year contracts renewable so far as the company has contracts to support number of staff. If rigs become idle then the workers on that rig can be laid off with a months salary in lieu of notice and are recalled when a contract is secured for that rig.

Characteristics Of Vapour Compression Refrigeration Cycles Engineering Essay This project is a study on a performance of a Vapor Compression Refrigeration System VCRC. The performance of this type of refrigeration system is being investigated by using different type of expansion valves or also known as throttling valves. I am using three types of valves upon conducting this study which are thermostatic expansion valve, capillary tube expansion valve and constant-pressure expansion valve. Performance of refrigeration systems will be investigated on two kind of surrounding which are closed and open surrounding. A light bulb will act as a heating load which will increase the inlet temperature of air on evaporation. This project also discuss about the ideal application of each type of expansion valves. Which mean, where, when and what application the three types of expansion valves should be used for a better result. Refrigeration is the action of cooling, and in practices this requires removal of heat and discarding it at higher temperature. Therefore, refrigeration is a science of moving heat from low temperature region to the high temperature region. Refrigeration can be defined as a process of achieving and maintaining a temperature below that of the surroundings, with the aim to cool some product or space to the required temperature. The most important applications of refrigeration are the preservation of perishable food products. Refrigeration systems also used for providing thermal comfort to human beings by using air conditioning. Air Conditioning refers to the treatment of air so as to simultaneously control its temperature, moisture content, cleanliness, odor and circulation, as required by occupants, a process, or products in the space. The subject of refrigeration and air conditioning has evolved out of human need for food and comfort, and its history dates back to centuries. In refrigeration system, heat must be removed from the space area specifically the goods or matter we need to cool and released to the environment. Nature of heat movement is, heat always moves from the hotter to the colder space. To transfer heat from hot region to cold region, special device is required. The device is called Refrigerator. Refrigerator is a cyclic device and it is using a fluid called refrigerant as a working fluid. As known there are two common methods for refrigeration which are natural and mechanical. In natural refrigeration, ice is used to generate a circulation of air around the blocks of ice. Therefore some of the heat from the circulating air is transferred to the ice thus cooling the air. This method particularly used in air conditioning application. As in mechanical refrigeration, refrigerant is used as a working fluid. Refrigerant is a substance that is capable to transfer heat that it absorbs at low temperatures and pressures to a condensing medium. Refrigerator is an example of mechanical refrigeration. In a refrigerator, coolant is cooled in a condenser; from there it flow to the evaporator, where air is cooled by contact with the coil. In vapor compression refrigeration cycle (VCRC), there are four major thermal process take place which is expansion, evaporation, compression and condensation. In all practical applications, the actual VCRC is lower than the ideal cycle. Compression Process on Compressor Low-pressure vapor of refrigerant enters the compressor and then being compressed and moved to condenser as a high-pressure vapor (superheated). Condensation Process on Condenser The superheated vapor of refrigerant is then travels through the condenser  which first cools and removes the superheat and then condenses the vapor refrigerant into a liquid refrigerant by removing additional heat at constant pressure and temperature. Expansion Process on expansion Valve The liquid refrigerant is then goes through the  expansion valve  which is also known as throttling valve where its pressure will abruptly decreases, causing  flash evaporation  and auto-refrigeration of, typically, less than half of the liquid. By undergoing expansion process, mixture of liquid and vapor refrigerant is produced at low temperature and pressure. Evaporation Process on Evaporator The cold liquid-vapor refrigerant is then travels through the evaporator coil or tubes and it is completely vaporized by cooling the warm air (from the space being refrigerated) and being blown by a fan across the evaporator coil or tubes. The resulting refrigerant vapor returns to the compressor inlet to complete the thermodynamic cycle. Vapor compression refrigeration system using high-grade energy such as mechanical work. It has more noise and wear is possible to occur in the compressor since moving parts is placed there. Coefficient of performance, COP of VCRC will decrease if the pressure of the evaporator decreases. Performance of VCRC is adversely affected at partial load. Vapor Absorption Refrigeration System Vapor Absorption Refrigeration System  uses a heat source such as solar and kerosene-fueled flame to provide the energy needed to drive the cooling system. Vapor absorption Refrigeration system is a popular alternative to regular  compressor refrigerators  where electricity is unreliable, costly, or unavailable, where noise from the compressor is problematic, or where surplus heat is available such as from turbine exhaust or industrial processes. Absorption refrigerators powered by heat from the  combustion  of  liquefied petroleum gas  are often used for food storage in  recreational vehicles. There are two type of vapor absorption refrigeration system that is single-effect and double effect. Single-effect absorption system is using neither ammonia or water while double-effect absorption system uses lithium bromide or water. Since the presence of ammonia or lithium bromide directly into a confined space which is full of human is dangerous, vapor absorption system which used this material should be placed outside the building to be cooled. However, vapor absorption that using ammonia or lithium bromide is not practical to be used on a building since it is impossible to pipe this material into the building. Therefore all absorption cycle equipment is then designed to chill water which is then piped into the building. The chilled water is piped through to fan coil units in each of the conditioned spaces. The application of vapor absorption system using water is therefore well suited to multiple room systems or large spaces requiring air conditioning. This is because absorption chillers are a well-established technology that can offer considerable advantages over conventional, mechanically driven, vapor compression chillers.   Absorption cycle is using a fluid pair rather than using a mechanically driven or an electrically powered compressor to be function. The absorption cooling cycle is driven by the heat source, such as gas burner, steam or hot water. This heat source is used to boil and drive the refrigerant out of the refrigerant/absorbent mixture. This mixture is normally termed solution.   The refrigerant is then going through a condenser to remove the latent heat of condensation, thus returning the refrigerant into liquid form. The refrigerant is then passed to the evaporator where evaporation process draws heat from the recirculating chilled water. The resulting vapor from the evaporator is attracted and absorbed by the absorbent solution to complete the cycle. If the flash vapor removed from the chamber, the liquid will cool to the required temperature. In practice, more than one cooling stage is usually present. Steam Jet Refrigeration system will become more economical when both steam and cooling water is readily available. Steam jet refrigeration system working start with the steam passed through a  vacuum ejector  of high efficiency to exhaust a separate, closed vessel which forms part of a cooling water circuit. The partial vacuum in the vessel causes some of the water to evaporate, thus giving up heat through  evaporative cooling. The chilled water is pumped through the circuit to air coolers, while the evaporated water from the ejector is recovered in separate  condensers  and returned to the cooling circuit. In  steam jet refrigeration systems, water can be used as the refrigerant. Water is perfectly safe for human. These kinds of systems were applied successfully to refrigeration in the early years of century. At low temperatures the saturation pressures are low (0.008129 bar at 4 °C) and the specific volumes are high (157.3 m3/kg at 4 °C). The temperatures that can be attained using water as a refrigerant are not low enough for most refrigeration applications but are in the range which may satisfy air conditioning, cooling, or chilling requirements. These systems also used in some chemical industries for several processes, such as the removal of paraffin wax from lubricating oils. The application of steam jet refrigeration system also including cooling a hot corrosive liquid efficiently to any desired temperature and the cooling of fruit juices instantaneously.Note that a steam jet refrigeration system is not suitable to be used when the required temperatures are below 5 °C. Steam jet refrigeration systems use steam ejectors to reduce the pressure in a tank containing the return water from a chilled water system. The steam jet ejector utilizes the energy of a fast-moving jet of steam to capture the flash tank vapor and compress it. Flashing a portion of the water in the tank reduces the liquid temperature. High-pressure steam expands while flowing through the nozzle 1. The expansion causes a drop in pressure and an enormous increase in velocity. Due to the high velocity, flash vapor from the tank is drawn into the swiftly moving steam and the mixture enters the diffuser. The velocity is gradually reduced in the diffuser but the pressure of the steam at the condenser is increased 5-10 times more than that at the entrance of the diffuser (e.g. from 0.01 bar to 0.07 bar). The main advantages of this system are the utilization of mostly low-grade energy and relatively small amounts of shaft work. Using Steam Jet refrigeration system will required negligible maintenance and a plant can be fabricated in any suitable material of construction. This is done by simply passing a small current through a closed circuit which is made up from two dissimilar metals. This type of effect is called the Peltier effect, and a refrigerator which is works using this kind of principle is called a thermoelectric refrigerator. The thermoelectric device is like the conventional thermocouple. It is using two dissimilar materials as a medium to transfer heat from one another. In a thermoelectric refrigerator, there are two junction between the two different metals used. One junction is placed in the refrigerated space and the other one is placed in the ambient surrounding. The temperature of the junction which is located in the refrigerated space will decrease when the current passed through and create a potential difference. An opposite effect will occur on the other junction. Under steady-state operating conditions, heat is transferred from the refrigerated space to the cold junction. This concept normally suited to recreational applications due to the weight of thermoelectric refrigerator which is light, compact and it is also insensitive to motion or tilting. This type of refrigerator also has no moving parts, and can operate directly from 12-volt batteries. Using thermoelectric refrigerator will give a advantages to the food and beverages. It will keep the food and beverages cold and dry. There also no wasted space for ice. After the Civil War, the South lay in ruins. Streets were lined with the lifeless bodies of Confederate soldiers whilst the buildings smoldered right down to their foundations. In an effort to restore the South to its former charm, the U.S. government plunged the country into what is now called the Reconstruction Era. The Puritans were people who had left England to live a pure and holy life in America. They obeyed the laws of the bible and were a very superstitious group of people. They believed that any behaviour that opposed the bible, even the tiniest thing, was of the devil and his witches. In 1950’s America there was a Senator called Joseph McCarthy who led the Un-American Activities committee. This meant that he was in charge of ridding the continent of anything that was deemed Un-American. It had not been long since the Second World War and all Americans were very paranoid about the rise of communism in some of the eastern European countries. Joseph McCarthy was not at all interested in putting people in jail but ‘naming names’- he wanted to know the names of those who were involved in communism. This process was soon to be known as McCarthyism. At the time of McCarthyism, Arthur Miller had a close friend who had been accused and was willing to ‘name names’- Miller was angry and began to write the crucible. The crucible is known as what is called an Allegory. This means that it is written to subliminally show something about one thing when it is being described as another. The crucible describes the Salem which trials in 1692 but is meant to show the complete similarities between it and 1950’s McCarthyism. Two of the main characters in The Crucible are John and Elizabeth Proctor. We learn at the beginning of the play that John and Elizabeth have a very difficult marriage. There is growth in the mistrust between them as we grow to learn that John has had an affair with the reverend’s niece: Abigail. After numerous events John and Elizabeth are sent to prison on the charges of witchcraft. Near the end of the play they meet for one last time to discuss their course of action. Faced with a difficult decision, John must choose between life and truth. This decision is difficult because he wishes to live and see his children grow up but he dare not lie about committing a crime he is not guilty of. He will do only what his wife wishes. But what is her wish? When her and John first meet in the prison she says to him â€Å"The children are well. Rebecca Samuels keeps them† she says this with the intent that John will feel a loss on the behalf of his children.

Nursing Effectivness Concept Paper Essay

Walker & Avant’s methodology (2005) is used to analyze the concept of the word effective; this provides clarity to the meaning of the concept and identifies its unique attributes, particularly in relation to medical treatment. Antecedents and consequences of presumed effective medical treatment are discussed followed by empirical referents, which aid in illuminating the concept of effectiveness. Assumedly, â€Å"effective† is an adjective most healthcare professionals and their patients would hope to use in describing the outcome of the treatment or the care provided. The purpose of this concept analysis is to understand that for physicians and nurses to provide truly effective treatment, it must attend to multiple needs of the patient, not just their illness. Not only is it important for healthcare providers to construct and implement a treatment plans, but also to involve patients in medical decision making about their treatment. To determine effectiveness, healthcare providers must assess how treatment of the illness effects the patient in addition to their own evaluation of treatment. The dictionary definitions of effective are (a) adequate to accomplish a purpose, producing the intended or expected result; (b) in operation or in force, functioning; (c) producing a deep or vivid impression; (d) prepared and available for service; and (e) a member of the armed forces fit for duty or active service (â€Å"Effective†, n.d. a). According to Stedman’s Medical Dictionary (2006), the medical definitions of effective are (a) the extent to which a treatment achieves its intended purpose; (b) A measure of the accuracy or success of a diagnostic or therapeutic technique when carried out in an average clinical environment. According to Wikipedia (n.d. b), the usage of effective includes (a) mathematics, can be used as a synonym of algorithmically computable; (b) physics, an effective theory is, similar to a phenomenological theory, a framework intended to explain certain effects without the claim that the theory correctly models the underlying processes; (c) heat transfer, effectiveness is a measure of the performance of a heat exchanger; (d) business/management, effectiveness relates to getting the right things done; (e) human–computer interaction, the accuracy and completeness of users tasks while using a system; and (f) medicine, effectiveness relates to how well a treatment works in practice. This concept analysis will focus on effectiveness in relation to treating an illness and patient outcome. As previously stated, it is important to consider how treatments can also have an effect on the patient. A patient’s illness must acutely be treated, but it is imperative to also set long term goals so the most effective and desired outcome can be achieved. Walker & Avant (2005) describe defining, or critical attributes as the characteristics that are most frequently associated with a concept. These attributes are key factors that must be present in defining the concept that is analyzed. Literature reviewed helped determine the characteristics attributed to the concept of effective in terms of nursing and medical treatment including, being free of disease or illness, decreased discomfort and suffering, scientific support, and improved quality of life. Another characteristic of effective nursing and medical treatment that should be considered is cost. Patients are sometimes informed about optional medical treatments that are available, but experimental and not yet approved by the FDA, nor covered by one’s medical insurance. There are many experimental treatments that have proven to be effective in curing illness but sadly, most cannot afford to pay for these treatments. This also applies if a person’s insurance does not cover a specific test, medicine, or treatment. A patient’s personal experience should also be taken into consideration; people with the same illness could receive the same therapy or treatment, but have different outcomes. An 11 year old boy presents to the emergency department with an obvious deformity to his left lower leg after falling off his bike, he is triaged and quickly seen by a physician for treatment. The physician puts a cast on his leg and informs the boy’s parents that his injury is not indicative of surgery and the bone should heal properly while casted. They are told to return in six weeks to have the cast removed and to evaluate the healing progression. The boy’s parents are then advised of activity restrictions, given a prescription for as needed pain medication and discharged home. They returned six weeks later and reported compliancy with the activity restrictions and he that only complained of pain the first couple days following the injury, which was managed well by the prescribed pain medication. The physician’s reassessment of the patient’s leg indicated optimal healing had occurred and the cast was removed, thus achieving the desired outcome. This case identifies that the patient’s pain was well managed and restrictions on activity were followed. The outcome identifies the patient is free of injury and discomfort, and quality of life was not affected, modeling all the critical attributes indicative of effective treatment. An 18 year old male is transported to a hospital via ambulance for an injury he sustained while playing in his high school’s football game. It was is reported to the healthcare team that he was tackled to the ground by an opposing player and Due to the risk of injury to his spinal cord and the patient complaining of sever back pain, radiological tests were performed. Tests showed in addition to the injury sustained during the game, he also suffered from some degree of spinal stenosis. The physicians explained a need for immediate treatment as it was imperative to surgically repair the damage to his spine by undergoing a spinal fusion. The physicians also inform them in order to achieve optimal healing and reduce the risk of further, irreversible long term damage, he will be unable to play football again. This news is very upsetting for the patient because not only is he unable to play again, it was his dream to make football a career and was already accepted to college on a football scholarship. This patient was discharged from the hospital a week after his surgery with outpatient physical therapy sessions if needed and a prescription for pain medication. The healthcare team explained to the patient, despite the possibility of mild back pain, the surgical treatment he underwent was effective. The concept of effective treatment related to this patient’s injury is that of a borderline case. Although the outcome of his treatment restricts his physical activity, healthcare providers were able to successfully treat his injury with surgical intervention. Pain medication was prescribed to decrease any discomfort or pain that may occur because from experience and scientific data, healthcare providers are able to recognize what treatments are proven successful to achieve the best outcome for the patient. A 17 year old girl, suffering from anorexia and bulimia, is admitted to the hospital for the fifth time in seven months. Every admission, including this one, she has received enteral feedings to improve caloric intake and hemodynamic status. The psychological status of this patient is also evaluated by a hospital therapist in addition to ongoing outpatient treatment for her eating disorder. The patient’s parents voice their concerns to the healthcare team about her deteriorating health status and frustrations with outpatient therapy not being effective. They are concerned because even though her health status improves with the treatment she receives in the hospital, outpatient therapy is not helping her maintain it. The physicians recognize with her parent’s concerns and agree she needs intensive therapy from an inpatient facility that specializes in eating disorders. However, the patient’s insurance will not cover extended inpatient treatment and is too expensive for her parents to pay out of pocket, so they must continue treatment that has been seemingly ineffective to her recovery. The outcome in this scenario is that of a contrary case of effective nursing or medical treatment. Although this patient is always acutely treated, she is not free of her disease because she continues to purge food or not eat at all. Her quality of life is also effected due to multiple hospital admits and her daily struggle with food. This patient’s outpatient therapy shows to be ineffective and sadly she continues to suffer with a n eating disorder. It is essential to discuss related framework to identify and clarify the meaning of an analyzed concept. Walker & Avant (2005) define antecedents as events that must occur prior to the occurrence of the concept. In order for a therapy or treatment to be effective, there first must be a person with an injury or illness who seeks medical care. After an individual has gone to an emergency department, urgent care, or physician’s office, they are evaluated by a healthcare team that includes a physician and nurse. Then, the healthcare team diagnosis’s the patient before deciding on the type of treatment warranted to treat their injury or illness. It’s imperative to include the patient in deciding treatment as â€Å"active patient involvement in medical decision making improves their quality of life and outcomes from treatment† (Egger, 1995. p. 384). When a type of treatment has been determined, it may require the patient to legally consent. The patient’s healthcare team is responsible for informing them about the treatment and any associated risks that may occur as a result. Once the patient’s treatment is completed they will be evaluated by the healthcare team, thus resulting in a patient outcome. The consequences, or events that occur as a result of, are the outcomes of a concept (Walker & Avant 2011). The consequences effective medical treatment are improved health status, decreased need for health services, increased patient satisfaction, and healing. All of these outcomes are desired by the patient as well as the healthcare team. These outcomes can have a huge influence on healthcare by providing data for healthcare providers and individuals researching treatment options. â€Å"Outcomes are the ultimate test of the effectiveness of medical care. Patient outcomes are clinical endpoints, functional status, general well-being, and satisfaction with medical care† (Coyle & Battles, 1999. p. 5). Empirical referents can be described as actual phenomena that by their actual existence or presence demonstrated the occurrence of a concept, in which is proven useful in instrument development (Walker & Avant, 2005). It is important to note that empirical referents measure and relate to the defining attributes of effective, not just the concept itself (Walker & Avant, 2011). Since effective (medical treatment) is subjective as well as objective, it is essential to consider both phenomena that occur with this concept. Improved health status is the most significant phenomena associated with effective health treatment, both subjectively and objectively, relating to the attribute of being free from illness or injury and quality of life. The healthcare team objectively considers a patient’s treatment as effective if it has cured illness, resolved injury, or improved quality of life. Scientific research provide healthcare providers with support in determining what medical treatment has proven effective. For patients, aside from being free of illness or injury, quality of life is typically the most important. There are many different treatments that cure illnesses, but sometimes alter a person’s quality of life. A patient can be in remission from cancer by receiving radiation and chemotherapy, but also causing a negative impact on their functionality, appearance, and general well-being. In order to determine how treatment has effected a patient’s quality of life, healthcare providers must formulate their own opinion based off of their knowledge of the patient and assess how the patient feels the treatment has impacted their life. Lastly, a patient’s use of medical services will be decreased or not needed at all. This analysis of the concept of effective medical treatment is defined as an outcome producing the intended or desired outcome. This concept has also shown to be complex, being that it is subjective as well as objective. Healthcare providers and patients must agree that the treatment was effective to the illness or injury. Effective medical treatment is achieved after a person with an illness or injury seeks medical attention and receives treatment for it. A review of literature on this concept concludes that to improve quality of life and patient satisfaction, the healthcare team must integrate the patient’s values and preferences about the intended treatment or therapy (Egger, 1995). Regular physical activity is important for a healthy growth, development and well-being of children, the British Heart Foundation has carried out research into the fact of living a healthy lifestyle, eating healthy can stop you gaining weight, which means reducing the risk of diabetes and high blood pressure. It can also help lower your cholesterol levels and reduce your risk of some cancers. Children who eat meal around the table with others are more likely to try different foods, therefore lowering the possibility of them becoming picky eaters. In order to eat healthily food should be selected from the various different groups to make up a nutritionally balanced plate. Games displays and the creation of posters can all assist with the promotion of a healthy lifestyle for young children. 4. Wellbeing is the state of being comfortable healthy and happy. In child it is necessary to evaluate children regularly to identify any children who may be displaying sings of emotional or social difficulties affecting their wellbeing. Children should feel comfortable enough to be able to express themselves in an environment that they perceive as positive and safe, and this should be encouraged. Wellbeing for families there are many public and private courses that run including the triple p programs (positive parents programs) which runs nationwide offering invaluable support to parents. Courses are available for different age groups of children and are delivered by experienced and appropriately trained practitioners. These courses have proved a lifeline for some parents who have been struggling in silence to conquer various emotional, physical or behavioural aspects of their children’s lives. This is a fantastic tool for improving parent’s wellbeing and offers an opportunity for parents to meet in secure non judgemental and confident environment where they can discuss their own issues that they are experiencing with practitioners and fellow parents †¦
